Scientists Reconsider The Role Of The Thymus In Adult Health
A small, fatty gland tucked behind the breastbone has become the focus of growing scientific debate over its role in adult health. Long regarded as largely expendable after puberty, the thymus is now attracting renewed attention as researchers investigate whether removing it could influence cancer risk, immune function, and long-term survival.
The thymus plays a critical role during childhood by training T cells, a type of immune cell that helps the body recognize and fight infections, abnormal cells, and disease. As people reach adolescence, the organ gradually shrinks and becomes less active. Because of this age-related decline, many surgeons have traditionally considered the adult thymus to be of limited importance.
New Evidence From A Harvard Research Team
In 2023, researchers from Harvard University and Massachusetts General Hospital published findings that challenged this long-standing assumption. The team analyzed outcomes among adults who underwent cardiothoracic surgery and compared more than 6,000 patients whose thymus remained intact with 1,146 patients whose thymus was removed during surgery.
The researchers reported that adults who underwent thymectomy were nearly twice as likely to die from any cause within five years, even after accounting for factors such as age, sex, race, and underlying conditions including thymus cancer and myasthenia gravis. They also observed approximately double the risk of developing cancer during the same period.
Importantly, cancers diagnosed in the thymectomy group appeared more aggressive and were more likely to recur than cancers diagnosed in patients whose thymus was preserved. Lead author David Scadden argued that these findings suggest the thymus continues to contribute meaningfully to immune health throughout adulthood.
Additional blood testing in a subset of patients appeared to support this interpretation. Individuals who had undergone thymectomy showed reduced diversity in T-cell receptors, an important feature of a healthy immune system. Broad T-cell receptor diversity allows the body to recognize and respond to a wider range of threats, including infections and cancer cells.
Why The Thymus May Still Matter In Adulthood
For decades, scientists have recognized the thymus as the primary training center for the developing immune system during childhood. Children who lose the gland early in life often experience persistently lower T-cell counts and weaker vaccine responses, underscoring its essential developmental role.
Because the thymus shrinks with age and gradually becomes infiltrated with fatty tissue, surgeons frequently remove it during heart and lung operations to improve surgical access. In some situations, including thymic tumors and severe autoimmune diseases, thymectomy remains a standard and often necessary treatment.
However, the Harvard study, published in The New England Journal of Medicine, raised the possibility that even an aging thymus continues producing new T cells. If true, the organ could play an ongoing role in maintaining immune surveillance against infections and cancer, challenging the traditional belief that it becomes biologically irrelevant after adolescence.
Yale Researchers Reach Different Conclusions
Not all researchers agree with the interpretation of the Harvard findings. In 2025, a team from Yale School of Medicine published a follow-up analysis using data from two large U.S. cancer registries: the National Cancer Database and the Surveillance, Epidemiology, and End Results (SEER) program.
The Yale researchers reviewed patient records spanning 2004 to 2022 and focused specifically on adults with small or localized thymomas who underwent thymectomy. Their analysis found no evidence of increased five-year mortality or cancer-related death compared with similar patient groups.
These results suggest that, at least in certain clinical situations, thymectomy may not carry the broad survival penalties suggested by the earlier Harvard study. Nevertheless, the authors emphasized that additional research involving longer follow-up periods and direct measurements of immune function will be necessary before definitive conclusions can be reached.
Why The Findings Conflict
The differing results from Harvard and Yale highlight the challenges of interpreting observational medical studies. Neither investigation was designed as a randomized clinical trial, making it difficult to establish direct cause-and-effect relationships.
Researchers note that differences in patient populations, underlying medical conditions, surgical indications, follow-up duration, and statistical methods may help explain the contrasting findings. It remains possible that factors unrelated to thymus removal contributed to some of the observed differences in cancer risk and mortality.
As a result, scientists caution against drawing firm conclusions based on any single study. Instead, the evidence should be viewed as part of an evolving effort to better understand the role of the thymus in adult health.
What This Means For Patients
Experts emphasize that the current research should not cause alarm among people who have already undergone thymectomy. Many individuals live for decades after thymus removal without experiencing serious immune problems, and the available studies do not prove that thymectomy directly causes cancer or shortens life expectancy.
However, the emerging evidence may encourage surgeons to think more carefully about removing the thymus when it is not medically necessary. In situations where preserving the gland is feasible and does not compromise treatment, maintaining thymic tissue may prove beneficial until stronger evidence becomes available.
Future studies are expected to follow patients over longer periods and include more detailed assessments of immune function. These investigations may clarify whether reduced T-cell production following thymectomy contributes to higher long-term health risks or whether earlier findings were influenced by other medical factors.
Renewed Interest In A Forgotten Organ
After decades of relative neglect, the thymus has returned to the center of scientific attention. Advances in immunology and long-term patient tracking are allowing researchers to revisit assumptions about the organ’s significance in adulthood.
Whether the thymus proves to be a major contributor to lifelong immune resilience or simply a marker of broader health processes remains uncertain. What is clear is that scientists are no longer viewing it as a useless remnant of childhood.
As research continues, this small gland behind the breastbone may reveal important insights into aging, cancer prevention, and the long-term maintenance of immune health. Its role in adult biology could be far more significant than previously believed.
