New Proposal Suggests the Foundations of Healthy Aging May Be Laid Before Birth
Longevity research has traditionally focused on extending healthy life later in adulthood by preventing or treating age-related diseases. However, a group of international researchers argues that this approach begins far too late and that the foundations of healthy aging may be established much earlier in life.
In a correspondence article published in Nature Health, the researchers propose a global initiative that would shift longevity research and preventive strategies to the earliest stages of human development—even before conception. They argue that understanding aging across the entire lifespan could fundamentally change how medicine approaches disease prevention and healthy aging.
A New Vision for Longevity Research
The proposed initiative, known as PROSPER—short for Pregestational and Pediatric Research for Optimal Healthspan and Early-life Resilience—aims to coordinate research, clinical trials, and medical guidelines that follow the biology of aging from the preconception period through childhood and into adulthood.
According to the authors, much of today's longevity research focuses on adults who have already accumulated decades of molecular and cellular damage. By that stage, biological processes such as chronic inflammation, metabolic dysfunction, and DNA damage may be more difficult to reverse.
The researchers suggest that identifying and addressing factors influencing long-term health much earlier in life could provide greater opportunities to promote healthy aging before disease processes become firmly established.
Measuring Biological Age in Early Life
The authors emphasize that biological age has a different meaning in children than it does in adults. Rather than reflecting accumulated biological damage, biological age during childhood may be better understood in terms of developmental timing, coordination of growth, and resilience to environmental influences.
Because of these differences, the researchers caution against applying biomarkers developed for adults to younger populations. Instead, PROSPER would support the development of age-specific biomarkers and models capable of measuring how growth, development, and early-life exposures influence long-term health.
The authors believe these tools could help identify which early biological changes are most strongly associated with disease risk later in life.
Focusing on Peak Health
The paper also introduces the concept of "peakspan," defined as the period during which an individual reaches their highest level of physical and biological functioning. Rather than focusing solely on extending lifespan or delaying disease, the researchers propose increasing the proportion of life spent in this optimal state of health.
To achieve this goal, the consortium proposes integrating multiomics data, clinical observations, and functional health measures collected throughout development. By analyzing these long-term biological trajectories, researchers hope to identify health risks earlier and design interventions that help individuals maintain optimal function for longer.
Shifting From Treatment to Prevention
The authors envision incorporating new diagnostic tools and preventive interventions into routine obstetric and pediatric healthcare. Instead of waiting until middle or older age to treat age-related diseases, clinicians could potentially improve long-term health by optimizing early-life environments, nutrition, and, where supported by evidence, safe pharmacological interventions.
Implementing this approach would require close collaboration among specialists in pediatrics, obstetrics, geroscience, epidemiology, and bioinformatics. According to the authors, bringing together these traditionally separate disciplines could provide a more complete understanding of aging from its earliest biological origins through later-life health outcomes.
At present, PROSPER remains a conceptual framework rather than a clinical program. Specific diagnostic strategies and interventions have not yet been established. If adopted, however, the initiative could guide future long-term cohort studies and clinical trials that follow individuals from before birth into adulthood.
The researchers argue that adopting a life-course approach to longevity research could improve both scientific understanding and clinical practice. By focusing on health much earlier in life, they suggest medicine may eventually move beyond treating age-related disease toward promoting healthier aging across every stage of the lifespan.
