A THC-Based Medication Significantly Reduced PTSD Nightmares in a Large Clinical Trial

THC-based medication may offer new hope for people with post-traumatic stress disorder (PTSD) who struggle with chronic nightmares. A large clinical trial led by Charité – Universitätsmedizin Berlin found that a cannabis-derived drug significantly reduced trauma-related nightmares in many patients. More than one-third of participants reported that their nightmares disappeared completely.

The findings suggest the medication could become a valuable addition to PTSD treatment, particularly for patients whose sleep remains severely disrupted despite standard therapies. While the drug did not improve every aspect of PTSD, researchers say reducing nightmares alone may substantially improve quality of life.

Why PTSD nightmares are difficult to treat

PTSD can develop after traumatic experiences such as war, serious accidents, physical or sexual assault, or natural disasters. In many people, the brain struggles to properly process these events, causing traumatic memories to repeatedly intrude into daily life.

During sleep, these memories often return as vivid nightmares that force people to relive their trauma in disturbing detail. Many patients wake suddenly in fear, making it difficult to return to sleep. Over time, repeated nightmares can lead to chronic sleep deprivation, worsen anxiety and depression, and interfere with recovery through psychotherapy.

Current PTSD treatment typically combines trauma-focused psychotherapy with medications such as antidepressants. Although these approaches can improve overall symptoms, they frequently fail to eliminate recurrent trauma-related nightmares. In Germany, as in many other countries, no medication is currently approved specifically for treating PTSD nightmares.

Researchers have therefore turned their attention to the brain's endocannabinoid system, which plays an important role in regulating stress, emotional memory and sleep, particularly rapid eye movement (REM) sleep, when most dreaming occurs.

How THC may reduce trauma-related dreams

Tetrahydrocannabinol (THC) is the primary psychoactive compound found in cannabis. It activates cannabinoid receptors throughout the brain and body that form part of the endocannabinoid system.

This system helps regulate fear processing, memory consolidation and emotional learning. Previous laboratory and early clinical studies suggested THC may reduce dream intensity and lessen the emotional impact of traumatic memories during REM sleep.

Researchers believe this may weaken the cycle of nighttime fear, making sleep less distressing for people with PTSD.

Cannabis-based medications are already prescribed in some countries for chronic pain, multiple sclerosis-related symptoms and certain forms of epilepsy. In Germany, access to medical cannabis expanded following legislation introduced in 2017, allowing researchers to more thoroughly investigate both plant-derived and synthetic cannabinoid treatments.

Inside the dronabinol clinical trial

The researchers evaluated dronabinol, a prescription form of THC extracted from cannabis and administered as oral drops.

The study enrolled more than 170 adults diagnosed with PTSD who continued to experience severe, frequent nightmares despite previous treatment attempts.

Participants were randomly assigned to receive either dronabinol or an identical placebo every evening before bedtime for ten weeks. Neither participants nor clinicians knew who received the active medication, making the study double-blind and placebo-controlled.

Throughout the trial, researchers measured nightmare severity using a standardized eight-point rating scale. They also monitored sleep quality, overall PTSD symptoms, depressive symptoms, side effects and participants' own assessments of their health and daily functioning.

Nightmares improved significantly

After ten weeks, participants receiving dronabinol experienced substantially greater improvement than those taking placebo.

Average nightmare severity fell by 3.7 points in the dronabinol group compared with a 2.2-point reduction among placebo recipients. Researchers said this represented a clinically meaningful improvement rather than merely a statistical difference.

More than one-third of participants taking dronabinol reported that their nightmares had disappeared completely by the end of treatment. Another 21 percent experienced at least a 50 percent reduction in nightmare severity.

Approximately five out of six patients receiving dronabinol also reported a significant improvement in their overall health.

Many participants described sleeping longer and more peacefully and no longer fearing bedtime. Although the study did not directly measure whether better sleep improved psychotherapy outcomes, researchers suggest improved nighttime rest could make daytime recovery easier.

Benefits and important limitations

The researchers found that dronabinol's strongest effects were on nightmares and sleep quality rather than on PTSD as a whole.

Although participants slept better, the medication did not produce statistically robust improvements in overall PTSD severity or depressive symptoms compared with placebo.

Nevertheless, clinicians emphasize that treating nightmares alone can be highly valuable. Persistent trauma-related nightmares are associated with poorer daily functioning, increased suicide risk and reduced response to psychological treatment. Reducing nighttime distress may therefore help stabilize patients while they continue longer-term therapy.

The medication also appeared relatively safe during the ten-week trial. Mild to moderate side effects, including dizziness, headache and increased appetite, occurred more frequently in the dronabinol group, but researchers did not attribute any serious adverse events to the treatment.

What comes next

Researchers caution that these findings apply only to short-term treatment under close medical supervision. They do not support self-medication with cannabis products.

Participants did not show clear signs of withdrawal after stopping dronabinol, and investigators found no evidence of problematic drug-seeking behavior during the study. However, longer clinical trials are still needed to determine whether patients develop tolerance over time, whether benefits remain durable and whether dependence risks change with prolonged use.

The trial was led by Charité – Universitätsmedizin Berlin in collaboration with the Psychiatric University Clinic of the Charité at St. Hedwig Hospital, the University Medical Center Hamburg-Eppendorf and the Central Institute for Mental Health in Mannheim. Researchers now plan to compare dronabinol with other treatments specifically targeting PTSD nightmares, evaluate different dosing strategies and identify which patients are most likely to benefit.

For now, the findings add to growing evidence that cannabinoid-based medicines may help relieve certain PTSD symptoms. However, experts stress that any future use should involve careful medical supervision, individualized dosing and integration with established psychological treatments rather than replacing them.
