A Constipation Drug May Help Clear Brain Fog After Depression, Early Trial Suggests
Many people who recover from depression continue to struggle with memory problems, poor concentration, and persistent brain fog long after their mood improves. Now, a new study suggests that a medication normally prescribed for chronic constipation may help improve these lingering cognitive symptoms.
The research, published in Psychological Medicine, found that the drug prucalopride improved memory and executive function in adults with a history of depression who were in remission. The study was conducted by researchers from the University of Birmingham and the University of Oxford and was supported by the NIHR Oxford Health Biomedical Research Centre.
Why a Constipation Drug Might Affect the Brain
Prucalopride is currently approved for treating chronic constipation. It works by activating the 5-HT4 serotonin receptor in the digestive tract, helping stimulate bowel movements.
However, these same serotonin receptors are also found in several areas of the brain involved in learning, memory, attention, and decision-making.
Because serotonin signaling plays a central role in depression, researchers wondered whether directly stimulating 5-HT4 receptors might improve cognitive function in people who remain mentally sluggish after recovering from depression.
Unlike many conventional antidepressants, which influence serotonin more broadly, prucalopride targets this receptor more directly and acts relatively quickly.
How the Study Was Conducted
The study included 50 adults with a history of major depressive disorder who had remained in stable remission for at least six months and were not taking psychiatric medications.
Participants were randomly assigned to receive either 2 mg of prucalopride or a placebo once daily for between seven and ten days.
Before and after treatment, they completed a series of standardized cognitive tests assessing executive function, working memory, verbal learning, and emotional processing.
These tasks are widely used in clinical research to detect subtle changes in cognitive performance.
The Drug Improved Cognitive Performance
Participants who received prucalopride performed better than those given placebo on tests measuring so-called "cold cognition"—thinking processes that involve reasoning, memory, and attention rather than emotional responses.
Compared with the placebo group, participants taking prucalopride completed the cognitive tasks both more accurately and more quickly.
Researchers reported a moderate improvement in accuracy (effect size z = +0.59) and faster response times (effect size z = −0.69).
The study did not identify any serious gastrointestinal side effects among participants during the short treatment period.
Why Brain Fog Matters
Lead author Dr. Angharad de Cates said that cognitive difficulties remain an under-recognized challenge for many people recovering from depression.
Even after mood symptoms improve, problems with memory, concentration, and mental clarity can continue to interfere with work, education, and personal relationships.
The findings suggest that targeting 5-HT4 receptors could offer a more direct approach to improving cognitive function than conventional antidepressants, which often produce inconsistent benefits for thinking and memory.
An Early Step, Not a New Treatment
The researchers emphasize that the study represents an early proof-of-concept rather than evidence that prucalopride should now be used to treat depression-related brain fog.
Senior author Professor Susannah Murphy noted that the trial was relatively small and lasted only one to two weeks. Much larger studies involving people with active depression and longer follow-up periods will be needed before any clinical recommendations can be made.
Future research will also need to evaluate long-term safety and determine how drugs targeting the 5-HT4 receptor interact with standard antidepressant medications.
Previous experimental research has suggested that 5-HT4 agonists may not only improve cognitive function but could also influence mood and potentially reduce vulnerability to depression. If future studies confirm these findings, the drug class may represent a new therapeutic approach that addresses cognitive symptoms alongside emotional recovery.
For now, prucalopride remains approved only for the treatment of chronic constipation. Experts advise people experiencing persistent brain fog after depression to consult their healthcare provider rather than attempting off-label use until larger clinical trials establish whether the treatment is both safe and effective for cognitive symptoms.
